Operational Policies

BILLING

Fees in this office are based on the procedure performed and on time spent with the provider. Payment of your bill is expected at the time of service. We accept cash, check and most credit cards. This office bills Medicare, Medicaid and most private insurers. It is our policy to bill your insurance carrier as a courtesy to you, although you are responsible for the entire balance at the time of service. If your insurance does not remit payment, the balance will be due in full from you.

Patients are billed on a monthly cycle for services received during the preceding 30 days. Charges not received in the business office before the filing date will appear on your next monthly statement. Bills are due in full unless other arrangements are made with our billing department. Questions concerning your bill will be answered by contacting our billing department. We believe that no one should be without medical or dental care especially in times of financial hardship, and we will attempt to work out a payment plan for you if needed. In the unfortunate event that you should be turned over to collections, you will be expected to pay for services at the time they are received. Co-payments for insurance and payment of services are due at the time of service.

INSURANCE

Our office automatically bills patient’s insurance companies provided we have the information necessary to file a claim. We need your social security number, all policy and group numbers, the subscribers name, company name and address and your employers name and address. You as the patient are responsible for payment of your bill regardless of the status of your insurance. Insurance payments vary with the type of policy and insurance carrier. Check with your carrier if you have questions regarding your coverage.

CANCELLING APPOINTMENTS

If you make an appointment with one of our providers and you are unable to keep it, please notify us as far in advance as possible in order to allow scheduling of other patients. We will be using reminder cards and phone calls to assist you in remembering your appointment. However, it is you responsibility to keep your appointments or notify us of cancellation and rescheduling. Continued missed appointments or failure to notify will result in restriction of appointment scheduling privileges.
